Scarletina is a rash caused by an infection somewhere in the body (usually the throat, but often other places) with group A beta-hemolytic Streptococcus germ. (Scarlet fever is the full blown syndrome of untreated Strep infection with a rash worse.) There are many strains of Streptococcus germ, which is causing this rash. The immune system antibodies that forms cross-react with not only parts of strep germ that act as a toxin but also normal tissues in the body's blood vessels. When this occurs in the skin, blood vessels are injured, dilate, and a characteristic red, sandpapery rash develops. It is often first noted in the area of underwear - where the skin is the warmest.
If you think this rash, even in a child with no complaint of sore throat, you should seek medical care for diagnosis and appropriate treatment with full course of antibiotics. Group A streptococcal infections are a small but definite risk of reaction with the other heart (rheumatic fever), which can cause permanent damage to heart valves or kidneys, causing dangerously high blood pressure ( acute glomerulonephritis), or the brain, causing a bizarre movement disorder (chorea, pronounced like Korea). The eruption scarletina is also something to watch in any child with chickenpox because it may indicate a secondary infection with this organism potentially poor quality. Call your doctor immediately if you see a rash scarletina with known or suspected chickenpox.
A week or two after the rash disappears you may notice some peeling of the skin especially of the arms and chest, and perhaps even peeling fingers and toes if the rash is severe. This is normal and safe
