You need to start with a therapist. They will check your status in order to start HRT.

It is really important to find a therapist gender. Try to get a referral if you can. Those who have more experience with trans-folk can save you time and money by analyzing your situation quickly. They are also more knowledgeable and helpful to get you started on the actual transition, explain the process, resources, what others found during the trip, etc.

The doctor may also ask. Do blood tests, you start on a low dose, and explain what to expect.

I first started going to a therapist and then submitted to an endocrinologist. More recently, I simply asked my doctor to prescribe them for me and him, he did it and did not need an official letter from a therapist. But most do. Usually they need several sessions with a therapist and a diagnosis of GID
You definitely want to do so under a doctors care, because it can be very dangerous. It can cause all sorts of health problems and they want to follow you with tests

IMHO If you are too afraid to see a doctor, then you are too afraid to go public and the transition.

HRT helps to feminize your body, but there is much more to do as well.

Getting the right mix is not something you can do overnight. There are a number of support groups at Yahoo that can help you. Some support the self-medication and do not take blood tests. Personally I have never done and to take regular blood tests.

As others have said, you must do so under the supervision of a physician. There are many complications that can be caused by hormones. These complications are not evident until after the damage has been done to the body. A doctor will monitor you for office examinations and laboratory tests.

Please do not self-medicate. I thought I understood the correct dosages and side effects. I stupidly self-medication for 3 years. I developed phlebitis, which is the formation of blood clots in the veins of the foot. T.
